Immunization Requirement Overview: Spring 2024
All students must complete the following two immunization requirements before the deadline below to avoid a late fee and registration hold.
Portal opens: Nov. 15, 2023
Completion deadline: Feb. 13, 2024
Requirement 1:
All students must submit proof of all required vaccinations or complete an exemption form.
Required Vaccinations:
- Measles, Mumps and Rubella (MMR): Colorado State Law requires all students to submit proof of two (2) doses each of the measles, mumps and rubella (MMR) vaccination or proof of immunity (titers).
- Meningitis (Men-ACWY): CU Boulder requires all students under the age of 23 living in university-operated housing to submit proof of the meningitis (Men-ACWY) vaccine given within the last five years and given after their sixteenth birthday. The meningitis (Men-ACWY) vaccine information must be completed by students living in a residence hall, Bear Creek, Athens Court, Athens North, Marine Court, Newton Court or Smiley Court.
Requirement 2:
All students, regardless of their vaccine status, must complete:
Required Forms:
- Tuberculosis (TB) Risk Assessment and
- Immunization Questionnaire related to Meningococcal Disease and Human Papillomavirus (HPV) vaccine.
Instructions to complete vaccine exemption forms
Students who cannot or who choose not to get a vaccine can complete and submit exemption forms
Please note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from CU Boulder and quarantine. The specifics may vary depending on the circumstances of the outbreak.
Step 1: MMR Exemption
Complete one of the forms below:
Medical exemption:
- Complete the Certificate of Medical Exemption (MMR). This form must be signed by a licensed physician and must include a date and a reason for exemption.
Non-medical exemption:
- Option 1: Complete the Certificate of Nonmedical Exemption (MMR). This form must be signed by a Health Care Provider licensed in the state of Colorado.
- Option 2: Complete the Online Immunization Education Module. A certificate of completion form will be made available at the end.
Step 2: Men-ACWY Exemption
Complete the Meningococcal Disease Information and Vaccine Waiver (page 2 of document).
Step 3: Upload exemption forms
Once you have completed the necessary forms for MMR and/or Men-ACWY exemptions, login to the MyCUHealth portal with your CU Identikey and password. Select the option for Immunization Requirements, then click on the Upload button on left hand side and follow the instructions to upload your form as an Immunization Document.

Login to the MyCUHealth portal with your CU Identikey and password, then select the option for Immunization Requirements.
Step 2:
Click Forms on the left hand side of the page. Select Required Immunization Questionnaire (the first link available). Answer both questions and hit the submit button.
Step 3:
Click Forms on the left hand side of the page. Select the Required TB Risk Assessment (the second link available). Answer the presented question(s) and hit the submit button.
